$25 Monthly card pin says invalid

I recently bought another monthly card and tried to enter it enter my phone and this time it said that it was an invalid code, i have never had any problems before and thought this to be strange. i dont have my reciept or the activation paper that came with it so i was unable to return it to the grocery store from which i had purchased the card. somebody please help, it is important to have my phone up and running correctly.
