I added the 200 minutes/31 day easy minutes plan on 8/23/2015. Today my phone went inactive. I don't understand why. How do I get my phone reactivated?

I am having the same problem but with an unlimited plan. I paid for service after the phone was deactivated and I received service for a day. Then, I tried to make a call the day after and was told that my phone had been deactivated again.
